   PLEASE NOTE:  

   This form is for residents to report a tree issue, SPECIFICALLY for trees located within the Town's Right of Way or Easements, or trees that are located within Town Park Districts or Parks. 

   The Town cannot evaluate or address trees that are located on Private Property.  Trees on private property are the responsibility of the Homeowner. 

   Residents will be contacted once their form is reviewed by Town Staff.  Thank you!

    Detail any existing obstructions located in the vicinity of the tree, such as power lines/poles, proximity to the street/driveway and structures such as a shed, deck, pool or fence.
